About the role
As a Registered Nurse in the HITH team, you'll deliver high-quality, evidence-based nursing care to patients recently discharged from hospital or requiring acute treatment in the community.
You'll work closely with medical officers, allied health, and other nursing professionals to deliver services including:
* Comprehensive nursing assessments and clinical observations
* Medication administration and reconciliation
* Wound care and pathology collection
* Client education, advocacy, and discharge support
* Coordination with multidisciplinary teams to ensure seamless care

* Current Authority to Practice as a Registered Nurse with AHPRA
