Holistic Support Worker
This role delivers comprehensive support to young people who are marginalized, excluded or disconnected from services. You will build trust with young people through consistent and meaningful engagement and link them to housing, health, employment and community resources.
About the Role
* Provide support to young people in line with Youth Services Model of care and associated Frameworks.
* Establish effective relationships with clients that encourage self-determination and participation.
* Deliver case management support to clients in accordance with TSA case management models and standards of practice.
* Evaluate and mitigate risks to ensure the safety and wellbeing of young people and adhere to organisational safeguarding policies and state mandatory reporting requirements.
