We are seeking an Enrolled or Registered Nurse who is self-motivated, has attention to detail and a passion for providing person centred care. You will be joining an existing nurse team of nurses who work collaboratively with doctors, allied health and administration staff. The position is offered as a part time or full-time contract for 12 months with the possibility to extend. There is no regular weekend requirement. Duties You will assist general practitioners in treating patients in a primary care setting with a strong focus on chronic disease management, reviewing care plans for patients with an eligible chronic disease and help to facilitate access to required allied health services to further support patients in their health management. You will also be required to support the doctors by assisting with minor procedures, immunisations, wound care and other duties as needed. Skills, experience and requirements Enrolled or Registered Nurse with current AHPRA registration and CPR certificate Outstanding communication and documentation skills with attention to detail Proven ability and commitment to patient services and continuous quality improvement Strong interest in chronic disease management Current drivers’ licence and Working With Children’s Check.
