WHO ARE WE?

Established in 1991, Danila Dilba Health
Service (DDHS) is an Aboriginal community-controlled organisation committed to
providing culturally appropriate, comprehensive primary health care services
and community development within the Biluru (Aboriginal) community of the Yilli
Rreung (Greater Darwin) Region in the Northern Territory.

ABOUT THE ROLE

The
Diabetes Educator will enable people living in the community to self-manage
their diabetes and optimise their health and wellbeing. The Diabetes Educator
will be responsible for the provision of high-level service to people in Danila
Dilba Health Services. The Diabetes Educator will contribute to the planning,
development, and implementation of programs/services with an emphasis on
continuous quality improvement.

WHAT YOU WILL DO

Client Care

* Work within a
large multi-disciplinary team in a collaborative manner to:
* Provide diabetes and self-management guidance to people with
pre-diabetes, Type 1 and Type 2 diabetes and gestational diabetes.
* Develop individualized diabetes plans for clients in collaboration
with multi-disciplinary team, with the consideration of cultural, physical,
cognitive and literacy levels of clients and families.
* Plan, develop, implement, and evaluate individual and group diabetes
education strategies for clients throughout the lifespan in collaboration with
the multi-disciplinary team.

SELECTION CRITERIA

Essential

* Qualifications
as a health professional (e.g. Nurse, pharmacist, dietitian) and current
registration with APHRA or meets eligibility requirements of governing body.
* Australia
Diabetes Educators Association (ADEA) Credentialled Diabetes Educator or
working towards credentialling.
* Experience
working with individuals with Type 1, Type 2 and gestational diabetes to
facilitate and support self-management including endocrinologist specialist
support.
* Proven
ability to independently plan, prioritise and complete a high-volume complex
workload within a changing environment.
* Knowledge
of the social determinants of health (environment, economic and social
inequalities) that have a profound impact on the health and well-being of
Aboriginal and Torres Strait Islander people.
* Proven
high level communication and interpersonal skills including with the ability to
constructively negotiate outcomes with a diverse range of people and service
providers.
* Proven
ability to initiate, implement and complete continuous quality improvement and
service development activities; and
* Demonstrated
high standard of written skills in the preparation of reports, general
correspondence and recording clinical notes and a clear understanding of
confidentiality.

Desirable

* Experience working in
an Aboriginal Community Controlled Health Organisation.
* Experience working in
a multi-disciplinary team.


IN RETURN WE OFFER

* 6 weeks annual leave.
* 5 days paid leave during end-of-year shut down
(additional to annual leave).

EMPLOYMENT IS SUBJECT TO

* Willing to undergo a Police
Check.
* Ability and willingness to
obtain a Working with Children Clearance (Ochre Card).
* Current driver?s licence.
* Current CPR certificate or
the preparedness to gain one within 6 months of commencement.
* Fully vaccinated against
COVID-19 or an exemption as listed on the Australian Immunisation Register
(AIR) be submitted.
