About the Role
We are seeking an experienced Pharmacist to join our team in a unique opportunity to enhance medication safety, optimise therapeutic outcomes, and strengthen continuity of medication management for our residents.
This position involves working on-site one day per week to support our Residential Aged Care Facility (RACF) in delivering high-quality, resident-centred care. Key functions include improving medication use and safety within the RACF, supporting day-to-day medication management, and providing accessible expert pharmacist advice.
Credentialing Requirements
To be eligible for this role, pharmacists must be credentialed for Medication Management Reviews (MMR). Pharmacists currently credentialed will be recognised as ACOP-credentialed on a temporary basis until further notice.
Pharmacists participating in this initiative must complete additional education to gain full ACOP credentialing via an Australian Pharmacy Council accredited training program or Recognition of Prior Learning pathway.
