Child and Family Health Nurse Role
About the Opportunity
We are seeking an experienced Child and Family Health Nurse (CFHN) to fill a crucial role. As the CFHN, you will serve as an entry point into community healthcare systems, supporting integrated multidisciplinary service responses based on individual client and family needs.
* You will utilize the Family Partnership Model as the foundation for nurse-family relationships and supportive counselling to deliver programs such as Universal Health Home Visiting and Safe Start.
* You will provide screening and surveillance programs for infants and children informed by the NSW Personal Health Record using evidence-based tools.
* You will offer opportunistic immunizations according to the NSW Childhood Immunization Schedule (0-5 years).
* You will actively promote, protect, and support breastfeeding.
* You will facilitate evidence-based groups delivered to meet child health, development, and parenting support needs of families within the community.
About You
We seek someone with:
* Registration with the Nursing and Midwifery Board of Australia (NMBA), demonstrating compliance with NMBA's Registered Nurse standards for practice and approved standards, codes, guidelines, and frameworks commensurate with the position.
* Post Graduate Child and Family Health qualifications, Authorized Nurse Immunizer, and Family Partnership Training. Note: Can be working towards or completed within 12 months of commencement.
* Demonstrated continuous quality improvement ability to comprehensively conduct assessments, develop nursing plans, provide safe, appropriate, and responsive quality nursing practice, and evaluate outcomes to inform nursing practice.
* Demonstrated competencies in and/or knowledge of the complexities and challenges involved in caring for individuals and their families in the specific area of employment.
* Demonstrated engagement in therapeutic and professional relationships, communication, and interpersonal skills and the ability to work within a multidisciplinary team in a rural health setting.
* Ability to think critically and analyze nursing practice, with a willingness to provide leadership, embrace change, and be a change agent in the coordination and delivery of care.
* Demonstrated commitment to own professional development to maintain, improve, and broaden knowledge, expertise, and competence, capability, and develop the personal and professional qualities required using research and evidence-based practice reflecting an understanding of contemporary matters.
* Class C Driver's License and willingness to travel for work purposes, which may involve driving long distances with the MLHD.
