Our Swansea site incorporates a 50 bed Residential Aged Care Facility, 18 independent living units, an Urgent Care Centre, 3 sub-acute beds and a variety of Home Care packages and services. Our Scottsdale site incorporates a 61 bed Residential Aged Care Facility and a variety of Home Care packages and services.

Position Purpose
The purpose of this role is to work with residents to assess, design, deliver and adapt exercise and physical activity interventions to prevent and manage chronic diseases, injuries, and other health conditions. Working as part of a care team providing person-centred care, the Exercise Physiologist focuses on supporting improvement of physical function, health and overall wellbeing through personalised exercise programs to contribute to the overall health outcomes of residents, aligned with the aged care standards and model of care.
The Exercise Physiologist also works with staff to actively contribute to the maintenance of good manual handling practices as they relate to work, health and safety processes and procedures.
Key Accountabilities
* Deliver quality exercise physiology services across all facilities
* Assess, plan, implement, and evaluate exercise physiology programs in consultation with multi-disciplinary care team
* Assist with assessments and reviews
* Establish and maintain accurate and up to date documentation according to compliance requirements
* Actively contribute to the maintenance of good manual handling practices as they relate to work, health and safety processes and procedures
* Collaborate with the care team to support continual improvement of processes and procedures
Selection Criteria
* A recognised degree in Exercise Physiology
* Eligible for membership of professional body for Allied Health practitioners
* Driver Licence and ability to travel across regional sites
* Demonstrated ability to deliver results: drives and delivers performance against a set of goals
* Ability to undertake research, assess findings, draw conclusions, and report
* Well-developed written/verbal communication skill and interpersonal ability aligned to our values
* Proactively self-managed with well-developed organisational and time management skills
