Our ADA Link team supports people who face barriers to accessing aged care, working closely with local communities, health providers and support networks to ensure people receive the guidance and connections they need. About the Opportunity We’re seeking a Community Connector to provide personalised, intensive support to older people who need help understanding and accessing aged care and community services. You’ll support individuals experiencing social isolation, homelessness, mental health challenges, substance use, family and domestic violence, or justice system involvement. You’ll build trusting relationships, listen without judgement and help people access aged care and essential supports. In this role, you will: Provide high‑quality information, support and assertive outreach to help older people access aged care and community services. Engage proactively with vulnerable older people, building rapport through flexible, person‑centred approaches. Support clients to navigate My Aged Care, connect with assessment services and access broader community supports to enhance their wellbeing and reduce barriers to care. Build strong local networks and partnerships to promote ADA Link services and strengthen community responses, so older people can access the support they need. Maintain accurate records, contribute to quality improvement and represent ADA Australia at community and stakeholder events. Identify service gaps and develop partnerships with Aboriginal and Torres Strait Islander peoples, Australian South Sea Islander communities, LGBTIQ communities and culturally and linguistically diverse groups. About You You’re a community‑minded professional who thrives on building trust, supporting vulnerable people and navigating complex service systems. You bring strong communication skills, a social justice mindset and the ability to engage with older people who may be isolated or unsure where to turn. You will bring: Qualifications in human services, social work, behavioural science, community services or equivalent experience. Experience working with older people or vulnerable groups. Knowledge of aged care supports, funding pathways and community services. The ability to interpret and apply legislation, policies and procedures. Strong problem‑solving skills, sound judgement and the confidence to work autonomously. Excellent written, verbal and interpersonal communication skills. High‑level computer skills, including Microsoft Office.
