About the Role
We are looking for a Wellbeing Clinician to facilitate a social prescribing pilot program in Gawler

Your Key Responsibilities:
* Provide non-clinical, person-centred social prescribing support through solution-focused appointments within participating general practices
* Conduct holistic strengths-based assessments exploring participants' social, emotional, cultural, and wellbeing needs, including experiences of social isolation and loneliness
* Conduct risk assessments and make referrals to specialist services as required for clients with complexneeds that may be outside the scope for social prescribing or require specialist intervention

What You'll Need to Succeed
* Relevant tertiary qualification in social work, allied health, community services, health promotion or related discipline
* Demonstrated understanding and application of trauma-informed, strengths-based, and person-centred practice.
* Demonstrated ability to support behaviour change through approaches such as motivational interviewing, coaching conversations, or goal-focused support.
