Clinical Nurse Consultant - Integrated Chronic Care

Key Responsibilities:
* Registered Nurses are required to have current Authority to Practice with AHPRA.
* 5 years full-time equivalent post-registration experience in the specialty field is a requirement.
* Approved postgraduate nursing qualifications in chronic disease management are necessary.
