About the Role
As a Compassionate Care Specialist, you will play a vital role in guiding individuals through their care journey, ensuring they receive high-quality services tailored to their unique needs and goals.
* You will be responsible for supporting a defined portfolio of clients within various funding subsets, with a focus on delivering exceptional customer service and facilitating clients' understanding of available program funding and services.
* Your key objectives will include providing information, support, and referrals; developing personalized care plans that address clients' unique care needs; and overseeing client budgets to ensure services are delivered in compliance with legislative guidelines.
This dynamic role offers the opportunity to work closely with individuals, helping them navigate their care options, develop tailored support plans, and access the services they need to thrive. You will also collaborate with service coordinators to secure appropriate service bookings that align with clients' identified goals.
Requirements
To succeed in this role, you will require:
* A minimum of two years' demonstrated experience in care management, working within the Disability, Aged Care, or broader Community Care sectors.
* Demonstrated experience supporting individuals through the development of care goals and personalized care plans.
* A current Driver's license (QLD) and formal training in Case Management through a recognized course/program.
