We are seeking a skilled and compassionate Case Manager to join our team. The ideal candidate will have a deep understanding of Housing First principles and the ability to provide holistic, strength-based support to individuals and families in need.

* A relevant tertiary qualification such as Social Work, Welfare, or Behavioural Science, and/or proven experience in a similar role.
* In-depth knowledge of Housing First Principles and their application in practice.
* Proven ability to assess individual needs in a family context and develop, evaluate, and update case support plans using trauma-informed practice.
* Understanding of complex social issues affecting families and individuals accessing homelessness services.
Main Responsibilities:
To provide high-quality case management services to clients, focusing on housing stability, mental health, and overall well-being.
To work collaboratively with multidisciplinary teams to identify and address client needs.
To assist clients in achieving their goals and promoting independence.
