Job Description
As a valued team member, you will provide high-quality social work services to patients, clients, and their families/carers. Your role involves working collaboratively with the multidisciplinary team to deliver excellent patient care.
Key responsibilities include coordinating patient care for optimal health outcomes, maintaining social work and case management responsibility, conducting early psycho-social assessments, care planning, education, and advocacy. You will also liaise with community agencies, General Practitioners, counselling services, and carer support.
