**The Role**

We are seeking a dedicated and compassionate Home Care Service Advisor to join our Northern team, covering Tweed Heads, stretching across to Ballina and Casino. In this role, you will provide tailored support to clients, ensuring they receive high-quality home care services that meet their individual care needs. Your expertise in case management and client advocacy will contribute to improving the quality of life for those in our care.

**Key Responsibilities**:

- Develop and implement care plans tailored to each client's care needs.
- Conduct client assessments, including initial intake and ongoing reviews.
- Coordinate and monitor the delivery of home care services.
- Liaise with clients, families, caregivers, and external service providers.
- Ensure compliance with regulatory standards and organizational policies.
- Advocate for clients to access necessary resources and services.
- Maintain accurate and up-to-date case notes and documentation.
- Respond promptly to client and family concerns or changes in circumstances.

Must be willing to travel across region as required.
- Minimum Cert 4 in Aged Care
- Proven minimum 2 years' experience in case management, ideally within Home Care or Aged Care
- Excellent communication and interpersonal skills.
- Strong organizational and problem-solving abilities.
- Knowledge of Aged Care Quality Standards, NDIS, or similar frameworks is desirable.
- Proficiency in Procura, Alayacare or similar software is desirable but not essential
- A current driver's license and willingness to travel as required.
