Job Title: Patient Care Specialist
You will have an opportunity to provide high-quality patient care that is compassionate, person-centered and responsive to individual needs. This role requires specific skills and knowledge in palliative care.
Key Responsibilities
* Develop and implement individualized care plans that take into account the unique needs and circumstances of each patient.
* Collaborate with interdisciplinary teams to provide comprehensive care that meets the physical, emotional, and social needs of patients and their families.
* Provide face-to-face, telephone and telehealth services to ensure equal access to all those with advanced life limiting illness.
Requirements:
* A relevant qualification in nursing (e.g., RN, RMN, RPN) from an accredited institution;
* Current registration with the Australian Health Practitioner Regulation Agency (AHPRA);
* Demonstrated experience working in palliative care;
* Strong communication and interpersonal skills.
