Bidwill Medical Centre is a well-established, mixed-billing medical centre committed to providing high-quality, holistic care to our diverse community in Bidwill. We are looking for an experienced and proactive Registered Nurse to join our team, focusing on managing our chronic disease and preventative health programs.
The Role
We are seeking a highly skilled Registered Nurse to take ownership of our patient care programs. This pivotal role involves working closely with our General Practitioners to improve patient outcomes across key areas.
Key Responsibilities Include:
- Chronic Disease Management (CDM): Conducting comprehensive patient assessments, developing, coordinating, and billing GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
- Health Assessments: Performing 45–49 year old, 75+ year old, and Aboriginal & Torres Strait Islander Health Assessments.
- Immunisation: Delivering a wide range of vaccinations, including National Immunisation Program (NIP), travel, and flu vaccines, ensuring strict cold chain management.
- Wound Care: Performing complex and routine wound assessments, wound care, and dressing changes.
- Treatment Room: Triaging patients, assisting with minor procedures, ear syringing, venipuncture, and stock/cold chain management.
- Quality & Administration: Maintaining accurate patient records, contributing to quality improvement activities, and supporting general practice administration as required.
What We Are Looking For (Essential Criteria)
The ideal candidate will demonstrate proven experience in the core functions of General Practice Nursing:
- Current AHPRA registration as a Registered Nurse (RN).
- Minimum of 2-3 years' experience working as an RN, with significant experience in an Australian General Practice setting.
- Valid certification for Immunisation Program for Healthcare Practitioners (or ability to obtain).
- Demonstrated experience in chronic disease management/care plan coordination (GPMP/TCA).
- Excellent clinical skills, including triaging, wound care, and patient education.
- High level of administrative competence, organisation, and attention to detail.
