About the Role:
We are seeking a highly skilled professional to lead and coordinate care in the Emergency Department.
* Provide leadership and coordination of care in a dynamic team environment.
* Work closely with a diverse group of healthcare professionals, including nurses, occupational therapists, physiotherapists, and social workers.
* Develop and implement strategies to prevent avoidable admissions, optimize patient outcomes, and ensure seamless transitions from ED to home.
This is a permanent, part-time position at 32 hours per week on a rotating, 7-day roster. The service operates between 7:30am and 9:00pm, 7 days a week.
Key Responsibilities:
Direct Clinical Care:
* Contribute to direct clinical care and provide discipline-specific assessment and intervention in patients' homes to support safe discharge.
* Play a pivotal role in both direct clinical care and professional supervision of your discipline.
Strategic Direction:
* Contribute to the strategic direction of the EDCC service by developing and implementing evidence-informed practices at an individual and team level across diverse patient groups.
About You:
* You will have advanced clinical skills and a minimum of 5 years' experience in the specified Allied Health discipline.
* Current registration with AHPRA is required.
* You must be able to independently develop and deliver comprehensive evidence-informed practices at an individual and team level across diverse patient groups.
* Demonstrated strengths in positively leading others and a solutions-focused mindset are essential.
* You should have experience initiating and managing quality improvement and/or research aligned with our strategic plan.
* Exceptional prioritization, time management, and problem-solving skills are required.
* Strong interpersonal, written, and electronic communication abilities are necessary.
* Commitment to ongoing professional development and lifelong learning is expected.
