The Team
Future Pathways is part of AV's approach to the Department of Families, Fairness & Housing (DFFH) state‐wide initiative to support Family Preservation & Reunification (FPR). The Future Pathways team complements a suite of family supports provided within the FPR model.
The Future Pathways programme is an innovative approach built on practice elements such as motivational interviewing, communication, skills building and goal setting to respond to identified needs and achieve real and demonstrable outcomes. Working collaboratively, the team seeks to address key intersectionality issues with families, children and young people (for example substance use, mental illness, low income, social isolation) as identified by each regional FPR response.
This Role
* Lead a team in supervision, reflective practice and case consultation meetings.
* Oversee and coordinate referrals ensuring the Future Pathways team works collaboratively with Child Protection Practitioners, services and families to ensure risk assessments and safety plans are maintained and tracked.
* Ensure the Future Pathways practitioners provide a targeted intervention in the family home at flexible times and when they are most receptive to learning; this may include early mornings or evenings.
* Fulfil program requirements regarding case recording, data collection, registration standards and other requirements, such as FPR evaluations.
* Monitor program performance and outcomes through the collection and analysis of service data and implement changes to the operation of the programme.
* Work in conjunction with the Program Manager and EBM Practice & Implementation to plan and implement responses in relation to programme, sector and service developments.
* Identify performance trends, best practices and opportunities for improvement.
* Contribute to the performance monitoring and reporting requirements of the Future Pathways programme.
* Ensure the Future Pathways practitioners strengthen family connections, promoting placement prevention and the safety of children, young people and parents.
Role Requirements
To be successful in this position you will need:
* A relevant tertiary qualification in Social Work, Psychology, Early Childhood Specialist and/or related behavioural sciences at degree level with relevant experience; or diploma level with substantial experience (refer to Position Description for overseas qualifications).
* The ability to demonstrate competency against the knowledge, skills and personal qualities outlined on page 2 of the Position Description – such as:
o Supervising multi‐disciplinary staff providing coaching and feedback.
o Leading reflective practice discussions and team meetings.
o Working with high‐needs complex families.
o Using a child‐focused, family‐centred approach.
o Developing intervention goals and plans relating to the risk of out‐of‐home placement.
o Utilising culturally sensitive practice with marginalised and minority groups.
o Knowledge of child welfare services.
o Understanding theoretical approaches (e.g., motivational interviewing and social inclusion).
o Knowledge of evidence‐based interventions.
o Experience conducting risk and needs assessments.
o Working within a casework framework.
o The ability to perform work outside of standard office hours to service the needs of clients.
o A current Victorian driver's licence, a Working with Children Check and preparedness to undergo Criminal History Check(s) prior to commencement.
