We are seeking a highly skilled and dedicated Aboriginal Health Rural Generalist Transitional Nurse Practitioner to provide comprehensive care to patients across a range of services.
* Develop and implement holistic care plans to address the diverse needs of patients, including those with acute and chronic presentations across the lifespan.
* Establish effective partnerships with multidisciplinary teams to facilitate integrated care and contribute to clinical excellence through research, teaching, and leadership.
* Work independently and collaboratively in an advanced nursing role, meeting the clinical demands within their defined scope of practice and formulary.
* Foster culturally safe relationships with priority population groups, including Aboriginal and Torres Strait Islander people, individuals with disabilities, and culturally and linguistically diverse communities.
Key Responsibilities:
