About The Role We are looking for a passionate AHPRA registered professional (Nurse, Occupational Therapist or Social Worker) to join our fast-growing team in South Australia. In this role, you'll manage relationships and oversee service compliance for a portfolio of clients who have chosen The CareSide to fully manage their Home Care Packages. Your Responsibilities Complete client assessments and care plansMitigate client challenges and facilitate a smooth home care experienceSupervise care deliveryReview client progress notes and ensure corrective actions are implementedMeet with prospective clients and explain the value proposition of joining The CareSideReport and manage incidents and hazardsDevelop relationships with clients and their families and proactively seek feedback to ensure we're meeting their service expectationsCollaborate with the scheduling team to ensure staff and client expectations are metChair regular team meetings with staff to support them in their day-to-day dutiesConduct performance reviews with your team Your Background We are looking for a compassionate, reliable and organised individual who will thrive in a collaborative environment while actively assisting aged care clients and providing professional advice.
