Disability Support Worker Job Description
We are seeking a skilled and compassionate Disability Support Worker to join our team. As a Disability Support Worker, you will play a vital role in empowering individuals with disabilities to live as independently and thoroughly as possible.
Key Responsibilities:
* Act as the first point of contact for new participants, families, and representatives, providing information about available disability services and guiding them through the intake process.
* Conduct initial assessments and screenings to determine participant needs and eligibility for services.
* Accurately collect, enter, and manage participant information and documentation in organisational systems.
* Coordinate with internal departments to facilitate seamless service commencement and participant onboarding.
Required Skills and Qualifications:
* Education: Certificate III or IV in Disability, Community Services, Social Work, Human Services, or a related field.
* Experience: Previous experience in an intake, customer service, case management, or administrative role within the disability, community, or health sector.
* Knowledge & Skills: Understanding of the National Disability Insurance Scheme (NDIS) and relevant legislation, strong knowledge of person-centred practice and principles, excellent verbal and written communication skills, high-level interpersonal and relationship-building skills, strong organisational and time management abilities, proficiency in Microsoft Office and client management systems.
