The team The Back on Track Team undertake meaningful outreach with young people who have not transitioned from year 10 to year 11 and/or 12 and are not currently meeting the requirements of the Education Act (2016). The primary purpose of the team is to support young people to navigate their barriers to learning to assist young people to re-engage in education or training. Back on Track provides access to: A multidisciplinary team, and connection to the broader Student Support team A government car for use when providing outreach Individual and whole-team professional learning Supervision and an extensive range of staff wellbeing services within DECYP About the Role To coordinate appropriate case management and reengagement strategies for young people in consultation with various stakeholders. To assist the Manager in the general educational leadership, management and administration of the regional team.
