About this role
We are seeking a Senior Aged Care Assessment professional to join our team. The successful applicant will provide leadership, training and support to clinicians, conducting assessments in hospital or community settings per Commonwealth guidelines and the Aged Care Act 1997.
Key responsibilities
* Conduct comprehensive client-centered evaluations involving clients, carers and providers.
* Lead multidisciplinary teams in assessing frail clients aged 65+ for residential, respite, community care and rehabilitation programs.
* Develop quality, evidence-based practice through effective communication and collaboration within the public health system.
Requirements
* Nurse Grade 7 - Must hold a Nursing qualification and maintain registration requirements with AHPRA.
* HP4 - Must hold a tertiary qualification and maintain registration requirements with AHPRA.
* Previous Aged Care Assessment experience is an advantage.
