Bupa Willoughby is a modern care home set in a lush, leafy suburb on the Lower North Shore of Sydney. You'll be close to the local community with neighbouring; school, church, parks, Chatswood shopping precinct, local shops and transport links nearby. It consists of 85 beds, with a variety of care units. Our primary goal is placing our focus on the health, lifestyle and well-being of our residents. The heart of the role
As a Care Manager, you'll lead with purpose - supporting your team and making a meaningful difference to the lives of our residents every day. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance.

Duties include
* Oversee the daily clinical operations of the care home ensuring our residents receive the best care available.
* Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.
* Review clinical incidents, complaints and changes, providing solutions to reduce any risks.
* Lead compliance with AN-ACC assessments.
* Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.
* Fostering a strong team environment.
* Prepare and conduct clinical governance audits.
* Provide insights and innovative solutions for continuous clinical improvement.
* Oversee medication management in accordance with resident needs and legislative requirements.
* Liaise with allied health, medical practitioners and other services to provide optimal resident care.
What will you bring?
* Bachelor of Nursing with current APHRA is essential.
* Post grad qualification in aged care highly regarded.
* Strong and effective leadership, communication and interpersonal skills.
* A person-centred care mindset.
* A passion for nurturing a strong team environment.
* Ingrained passion and empathy for our residents.
* Proven track record in delivering quality clinical care within an aged care or acute setting.
* Excellent clinical assessment and triaging skills as well as strong care planning capabilities.
* Proven and demonstrated experience in fostering a reporting culture balanced with your effective investigative skills.
* An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes.
