Stroke Care Coordinator
About the Role:
This is a highly rewarding opportunity to lead the assessment, planning, and evaluation of stroke care for our network, ensuring compliance with best practice and achieving a coordinated patient journey from admission through to community reintegration.
Facilitating coordinated care for stroke patients presenting to health facilities within the Network, you will be responsible for leading multidisciplinary teams in the delivery of high-quality patient care.
Key Responsibilities:
* Developing and implementing evidence-based stroke care pathways and standards to ensure consistency across the network.
* Liaising with clinicians, managers, and other stakeholders to ensure seamless transitions of care and effective communication.
* Collaborating with local health districts and other networks to identify opportunities for service improvement and implementation.
* Monitoring and evaluating the effectiveness of stroke care services, identifying areas for quality improvement and developing strategies to address these.
Requirements:
