As an Aged/Disability Support Worker, you will play a vital role in assisting participants to live life to the fullest by delivering personalized care within clients' homes and SIL homes.
About the Role:
* Provide personal care, meal preparation, and maintain the home environment to promote independence and empowerment.
* Support clients' health by managing medication, appointments, and transportation to community activities.
* Maintain clients' dignity and independence through personal care and follow care plans.
Key Responsibilities:
1. Support participants in achieving their goals and living the life they choose.
2. Assist with daily activities, meal preparation, and personal care to promote independence.
3. Follow care plans, document progress, and monitor client assessments.
4. Promote overall well-being by providing transportation and accompanying clients to community activities.
Required Skills and Qualifications:
* Certificate III or IV in Disability/Aged Care/Individual Support.
* Valid Driver's License with a registered Vehicle.
* Passion for working in the Disability sector with strong understanding of its dynamics.
* Previous experience in a similar role is preferred.
* Excellent interpersonal and communication skills.
