About the Opportunity:
We seek skilled Housing Support Practitioners to join our Service Delivery Team, working closely with veterans and their families to develop tailored housing goals and case management plans.
Key Responsibilities:
* Provide comprehensive support to clients experiencing homelessness or at risk of experiencing homelessness
* Foster active engagement from clients in identifying and implementing individualised housing goals
* Apply best practice knowledge and skills to engage, support, and empower clients to overcome homelessness barriers
* Case manage clients through assessment, planning, and coordination of housing-related services
* Assist clients with applications for social and community housing, private rental options, and tenancy sustainment strategies
