The Geraldton Regional Aboriginal Medical Service (GRAMS) - Gascoyne Outreach Service is seeking a dedicated ITC Indigenous Outreach Worker to support Aboriginal and Torres Strait Islander clients by ensuring they receive timely and comprehensive care for chronic health conditions.


Key Responsibilities

1. Initiate patient recalls via letters, phone calls, or home visits.
2. Follow current policies and procedures regarding follow-ups and contact attempts.
3. Work closely with the Aboriginal Liaison Coordinator to ensure patients attend appointments with specialists or other health services. This includes arranging transport, ensuring proper documentation, and providing feedback to GRAMS.
4. Coordinate with the GRAMS Home Visit Team, Aboriginal Health Workers, and reception staff for patient follow-ups and appointments.
5. Transport patients to their specialist appointments, both locally and in metropolitan areas.
6. Contribute to GRAMS organizational goals and adhere to Best Practice Standards.
7. Support the Closing The Gap commitment.
