Remuneration value up to $172,272 p.a. comprising salary between $5,699 p.f. - $6,603 p.f. (L4 - L9) employer superannuation contribution (up to 12.75%), annual leave loading (17.5%), professional development allowance, professional development leave 1.6 weeks p.a.
About the Care in the Right Setting (CaRS) Program
The CaRS program aims to improve access to health care and reduce the demand on hospitals by improving flow and promoting the right care in the right time for the right outcome. The CaRS team includes Hospital In The Home (HiTH), Geriatric Evaluation Management / Rehabilitation – Hospital In The Home (GEM/R-HiTH), Residential Aged Care Facility Support Service (RaSS), and Geriatric Emergency Department Intervention (GEDI) care providers.
Service Description
* GEM/R-HiTH: Provides care to admitted inpatients in their homes or suitable accommodation. Service led by a geriatrician and rehabilitation physician, facilitating multidisciplinary community-based care for a seamless transition home. Supports a range of patients, including orthopaedic, stroke, respiratory, and chronic conditions, through short-term rehabilitation aimed at maximizing functionality in their own environment. Staff includes medical staff, nurses, physiotherapists, occupational therapists, social workers, dietitians and speech pathologists.
* RaSS: Provides care in partnership with residential aged care facilities and GPs to provide care in nursing homes.
