Job Description
We are seeking a Support Worker to provide individualised support services to customers in their own homes. The successful candidate will work as part of a team to enable customers to remain living independently and connected to their local community.
The role involves providing personal care, domestic assistance, and clinical care, including administering medication (training provided). You will also be responsible for manual tasks such as hoisting, social support, and regular communication with the local team.

* Social support, including assistance with trips in and around the community
Requirements
* A valid driver's licence and a reliable vehicle
* Ability to facilitate people to develop and sustain their own social connections and relationships
* Excellent communication skills and a can-do attitude
* Availability to attend mandatory and paid training during the week
* Current NDIS Worker Screening check or willingness to obtain one
