Job Title: Community Support Worker
Job Description:
* Support customers with identified wellbeing needs, strengths, and goals by developing individualised person-centred support plans.
* Assist with skill development, transport, social activities, finances, appointments, cooking, medication, and personal care to improve customer independence.
* Encourage participation in social, leisure, sporting, educational, employment activities etc. to promote well-being.
* Keep accurate and up-to-date records using online systems and electronic databases.
* Develop and maintain professional relationships with customers, families, caregivers and stakeholders.
Required Skills & Qualifications:
* Minimum Cert III in Disability, mental health, Peer Work or equivalent required.
* Valid NSW Drivers License (Class C) with ability to drive large cars and/or vans.
* Ability to obtain an Australian Federal Police Check, NSW WWCC and NDISWC prior to commencing.
* Strong set of values guiding your work incorporating passion, integrity & respect.
* Excellent computer skills with proven ability to use Microsoft Office and customer relationship management systems.
