About The Role
A concord hospital with over 70 years of heritage in caring for the veteran community now services local communities of Concord, Strathfield, Burwood and beyond. This position completes intake, scheduling, data collection and administrative duties within ACC&R.
Responsibilities
Complete intake for multiple ACC&R teams, applying eligibility criteria and directing referrals to the correct team within SLHD.
Schedule communications with clients, carers and referrers regarding teams' eligibility and availability.
Manage communication via email and phone with external providers for all teams involved in the Services at Home brokered services contract.
Enter data on client services into spreadsheets and compile reports to assist the contract manager in ensuring KPI's are met.
Ensure paperwork taken by assessors on visits is printed in a timely manner to enable those visits to be completed effectively.
Ideal Candidate
Understanding of and/or experience in the health and aged care service sectors.
Previous experience in waitlist management and booking systems.
Currently holds a valid NSW driver's licence (P1 and P2 licence acceptable).
