We are seeking a Program Officer to empower individuals with disabilities to live life on their own terms.
In this pivotal role, you will be instrumental in delivering our services in Canberra, supporting people experiencing temporary health challenges to remain living independently in their own homes.
**Key Responsibilities**
- Process new client referrals and coordinate initial onboarding or further referrals to other support systems.
- Conduct thorough assessments against eligibility requirements, ensuring a person-centred approach to care needs.
- Develop comprehensive reports to support program management and adherence to program guidelines and contract requirements.
- Foster partnerships with families and supporters to ensure seamless service delivery.
- Collaborate with key stakeholders to ensure consistency in program delivery and promote a strong service culture.
- Manage, monitor, and report on incidents and complaints effectively.
**Requirements**
- Relevant qualification in Community Services, Disability, Aged Care, or Business.
- Experience in disability, aged care, or community services is essential.
- Strong administrative skills, including proficiency in information systems and data management.
- Excellent verbal communication and relationship-building skills are crucial for success.
- Ability to manage competing priorities and deliver quality outcomes within given timeframes.
- Proficiency in computer literacy and ability to analyse, review, and present data.
- Current unrestricted Australian Driver Licence is mandatory.
