About the role

This role sits within Barwon Health's Youth Continuing Care Service (Youth CCS) and works closely with the Youth Mental Health and Wellbeing Service (YMHWBS) to support young people aged 12–25 years experiencing moderate to severe mental health and alcohol and other drug (AOD) challenges.

The Youth Mental Health and Wellbeing Service is part of Victoria's mental health reform and is designed to provide earlier access to specialist mental health care for young people, bridging the gap between primary care and acute services. Working within this model, Youth CCS provides longer-term treatment and support for young people with complex and persistent needs, including dual diagnosis, homelessness and social vulnerability.

In this role you will deliver evidence-based mental health and AOD care, including assessment, outreach, treatment and ongoing care coordination across the region. You will work as part of a multidisciplinary team under clinical and psychiatric guidance, partnering with families, GPs and community organisations to support young people on their recovery journey.

The service operates across the community and participates in a rotating 7-day roster with day, evening and weekend shifts, ensuring responsive and accessible support for young people when they need it most.

Key Responsibilities
* Deliver evidence-based mental health and AOD assessment, treatment and care coordination for young people aged 12–25.
* Provide flexible, outreach-based support across community settings, including engagement with young people experiencing complex needs such as dual diagnosis or housing instability.
* Collaborate with families, GPs and community partners to develop integrated care plans that support recovery and wellbeing.
What We Are Looking For
* Passion for supporting young people experiencing mental health and AOD challenges to achieve recovery and stability.
* Strong skills in youth mental health assessment, risk management and trauma-informed therapeutic engagement.
* A collaborative and adaptable approach, with confidence working across multidisciplinary teams and community-based settings.
