With the implementation of the new aged care Support At Home program, we are excited to recruit for a Clinical Care Partner (Registered Nurse). The position is preferably full time, however, a reduced full time equivalent may be considered. The position is based in our Warrnambool office.
Clinical Care Partners perform an integral role in ensuring individualized services are developed and delivered to customers based on their assessed clinical care needs, in consultation with customers, their family, or guardians and key health professionals. Clinical Care Partners are responsible for conducting comprehensive nursing assessments and developing, monitoring & reviewing individual care plans and associated health care plans.
While qualifications and experience are important, equally as important is who you are as a person. We are looking for someone with a passion for relationship building and supporting customers.
