What You'll Be Doing
The Keeping Well in the Community Care Navigation and Coordination (KWIC CNCC) Program is an integrated care initiative within South Western Sydney Local Health District (SWSLHD) empowering clients with the tools, support and motivation needed to independently manage complex chronic and psychosocial conditions in the community.
Our primary objective is to minimise avoidable hospital presentations and unplanned admissions, while expediting discharge from acute care settings. By facilitating and coordinating access to tailored care, non-clinical supports, and aiding informed/shared decision-making, we aim to comprehensively address a spectrum of unmet health and psychosocial needs to ensure clients can 'keep well in the community' for longer. Clinicians will provide assessment, screening, care navigation, care coordination and health coaching to identified clients in line with the NSW Health Strategic Framework for Integrating Care. Clinicians will deliver safe, compassionate, quality, person centered and integrated care, and work in collaboration with the client and families/carers, using a mostly telephone-based service model.
The registered Nurse, using refined clinical skills and exercising autonomy in decision making, plays a vital role in the KWIC CNCC program. Expectations center around maintaining manageable caseloads, while ensuring the provision of safe, quality care to individuals with chronic diseases, and complex care needs. This requires close collaboration with fellow clinicians, and the multidisciplinary team to streamline service delivery. The position demands diligence in clinical assessment, precise referral procedures, and seamless coordination of health and community services to optimise patient outcomes, and satisfaction.

SWSLHD Primary & Community Health provides a comprehensive range of community-based services designed to improve and maintain the health and wellbeing of individuals and communities across South Western Sydney.
These services include prevention, early intervention, assessment, acute and post-acute treatment, health maintenance, and continuing care. The team supports diverse populations, offering specialised programs for children, young people, older adults, and at-risk communities.
With over 800 dedicated staff, including Aboriginal Health Workers, nurses, doctors, and allied health professionals, the service ensures high-quality care close to where people live.

How To Apply

* Current Authority to practice as a Registered Nurse with the Australian Health Practitioners Regulation Agency and demonstrated recent experience as a Registered Nurse working with chronic and complex clients.
* Proven ability to engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs and undertake comprehensive health care, psychosocial assessment, formulate and implement appropriate phone-based intervention for clients and their families/carers.
* Demonstrated experience in facilitating education, health literacy approaches and self-management support for clients with complex healthcare and psychosocial needs.
* Demonstrated effective interpersonal and communication skills, including written, verbal and computer skills and an ability to work independently and collaboratively in a team-based environment.
* Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
