ABOUT THE ROLE
The purpose of this position is to lead Footprints Aged Care Assessment Service (ACAS). The ACAS is designed to deliver client-centred, comprehensive, fair, and timely assessments, empowering older Australians to access Commonwealth-funded aged care services. As a newly established service, the SAS will provide both home and comprehensive assessments targeted to meet the specific needs of senior Australians.
KEY RESPONSIBILITIES
Program Management:
* Drive the implementation of the ACAS in relevant aged care planning region (s).
* Manage the ACAS program to ensure successful compliance with funding guidelines, contracts, and legislation.
* Support the internal transition of the Regional Assessment Service to the ACAS.
* Ensure contractual requirements are met as per the Department of Health and Aged Care contractual requirements
* Support the reforms within Aged Care legislation, supporting employees to understand these reforms and implications for service delivery and clients.
* Investigate client complaints and other feedback, including escalation and management of such in line with internal policies/procedures, and regulatory/contractual requirements.
* Reporting, investigation, and management of program incidents in line with internal policies/procedures, including escalation and reporting in accordance with regulatory/contractual guidelines.
* Develop and maintain sustainable networks and partnerships with key stakeholders and groups to enhance program and services referral base and facilitate successful outcomes for both clients and Footprints.
* Attend internal and external meetings, including committee, peak body, and funding body meetings.
* Maintain oversight of internal and external communications which impact program (e.g., changes to policies/procedures, internal and external reform implementation, etc.), and ensure effective communication of same to team.
* Produce correspondence and reports, including program evaluation and activity reports commensurate with internal reporting, program guidelines and government contracts.
* Ensure the consistency, timeliness and quality of data collection, documentation, assessment, review and evaluation of programs and services, in accordance with internal reporting, program guidelines, quality standards and record keeping requirements.
* Drive the ongoing development, implementation, and evaluation of program/service practice, ensuring continuous quality improvement and compliance with relevant legislation and accreditation standards such as the Aged Care Assessment Quality Framework.
* Contribute to the writing of policy, procedures, and guidelines in accordance with program and organizational requirements.
* Maintain regular feedback and consultation with the Manager Single Assessment Service.
* Provide leadership, coaching, training, and support to team members who work in the Single Assessment Service ensuring collaborative approach in implementing and achieving seamless program and service delivery.

* Participate in monthly professional supervision sessions, and ongoing learning and development activities.
* Promote and maintain a safe and healthy workplace, ensuring adherence to WHS policies and procedures.

KEY SELECTION CRITERIA
Essential
* Relevant tertiary qualification e.g., social work, social science, human services or related discipline, and/or significant experience in community service provision in a leadership role.
* Demonstrated understanding/experience of the provision of community-based support services for the aged which reflects a person-centred approach that places clients at the centre of care with a focus on dignity, respect, choice, and self-determination,
* Demonstrate knowledge and experience in client lead support reflecting a person-centred approach, incorporating assessment, case planning, monitoring and evaluation processes.
* Demonstrated knowledge and understanding of Aged Care legislation and current reforms\
* Demonstrated leadership experience and the ability to work in a team, with a focus on employee support/development and performance management.
* Demonstrated knowledge and/or experience in working with a diverse client group including people who are ageing, people experiencing mental illness, people experiencing homelessness and at risk of, and people with disability and their carers/families.
* Demonstrated experience in community networking and working collaboratively with external service providers to plan and provide consistent, responsive, and appropriate client focused services.
* Demonstrated experience in community and government networking to promote partnerships and organisational growth/capacity to respond to reforms and new opportunities.
* Ability to network with key services in the sector to grow and enhance new referral pathways and foster new professional relationships for program growth.
* Ability to accurately monitor organisational data ensuring both service and budget targets are met.
* High-level interpersonal, written, and verbal communication skills and the demonstrated ability to work effectively in a team environment.
* Excellent time management skills and the ability to multi-task and prioritise.
* Computer literacy in relation to data analysis and professional report writing and the ability to effectively manage a client database. Ability to work remotely from a team when required, and ability to utilise virtual meeting platforms e.g., Microsoft Teams, Zoom.
* Current class C driver license.
* Ability to use your own comprehensively insured vehicle.
* National NDIS Worker Screening Check or current Yellow Card.
Desirable
* Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist (or other clinical or allied health qualification) with current AHPRA Registration or Social Worker with eligibility for AASW membership
* Current First Aid and CPR Certificate.
