Care Partner - Home Care Packages

We have an opportunity for a skilled Care Partner to provide care management for a designated caseload of Support at Home (SAH) participants as part of our Home Care Packages Team. This includes onboarding new participants, developing and reviewing individual care plans, and coordinating a range of services to meet complex care needs.
Care Partners play a vital role in promoting the safety, health, and overall wellbeing of older people. By delivering person-centered, high-quality care management, they ensure that services align with each participant’s preferences, cultural background, and support requirements, empowering them to live independently within the community.
This role combines community visits with clients and time in a dynamic office environment.
Essential Criteria
* qualifications in Social Work, Allied Health, Community Services, or a related qualifications will be considered based on the Support at Home Manual scope of acceptable qualifications.
(including Registered or Enrolled Nurses with current AHPRA registration – Division 1 or 2)
* You’ll be open to travel, balancing community visits with clients and time in a vibrant office environment
* Understanding of Home Care Packages/Support at Home, My Aged Care, and Aged Care Quality Standards
* Strong commitment to supporting older adults to remain living independently in their homes
* Well-developed communication, planning, organisational, and time management skills
* Proficiency in Microsoft Office and participant management systems
* Demonstrated ability to build and maintain effective working relationships with both internal teams and external service providers
* Current and valid First Aid and CPR Certificate
* A current Victorian Driver’s Licence
Desirable Criteria
* Experience in managing Home Care Packages/Support at Home or working within government-funded health or social service programs
* Familiarity with Carelink Plus or similar client management software
* Demonstrated experience working with diverse aged care populations, including those with specialised or complex care needs
* Background in assessment, care planning, and service coordination for community-dwelling older adults
* Proven ability to negotiate and liaise effectively with external community service providers
