Just Better Care Cairns are looking to welcome a experienced Aged Care Coordinator to our team. You will be someone with a can-do positive outlook and enjoy working within a small team.

Selection Criteria

Essential

· Certificate IV or Diploma or above in Individual Support, Disability and Ageing or equivalent.

· Previous experience in a Case Management role/Care Partner role.

· Previous experience supervising, supporting and training staff.

· Ability to work flexibly to respond to changing participants' needs.

· Ability to coordinate services according to relevant program guidelines.

· Experience of working in a multi-disciplinary team.

· Ability to develop and maintain links and supportive networks with specialist services.

· Knowledge of continuous quality improvement principles.

· Excellent people, problem solving, and conflict management skills.

· Ability to work as part of a team and maintain good working relationships

· Experience maintaining computerised participant information systems.

· High level administrative and organisational skills.

· High level of written and verbal communication skills.

· Demonstrated ability to contribute positively to the team.

· Availability for on call support

· Current:

o Criminal Record Check

o NDIS Worker Screening Check

o Working With Children Check

o Drivers Licence

o First Aid Certificate and CPR Certificate Tips: Provide a summary of the role, what success in the position looks like, and how this role fits into the organization overall.

Principal Accountabilities

· Manage day-to-day activities of the service.

· Provide supervision, support and direction to support staff.

· Ensure that a quality service is provided to people we support

· Ensure the ongoing development of flexible and innovative services through continuous improvement.

· Ensure that care management activity is delivered at least monthly to participants.

· Ensure care plans are developed in partnership with the participant and their representative/s.

· Ensure care plans are monitored and reviewed on a regular and ongoing basis (i.e. at least once every 12 months) and/or in response to changes in participant needs, services, circumstances and goals.

· Reviewing agreements where participants care plans change.

· Ensure services are delivered in line with the person centred and consumer directed principles.

· Assist participants to access other alternative services where the services are no longer meeting their needs, including facilitating transitions in care where applicable.

· Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery and to ensure standards are met in accordance with program objectives.

· Monitor, review and evaluate ongoing care discussions, case conferencing with relevant team members and/or external service providers.

· Support participants to make informed decisions and integrating reablement approaches.

· Monitoring the wellbeing of clients and informing Clinical Care Partner of any concerns or changes and taking action within the scope of the participant's individual care plan.

· Provide advocacy where necessary on behalf of the participants.

· Provide independent advice, information and resources to participants.

· Ensuring participant views, rights and concerns are heard and escalated.

· Assisting participants with providing complaints and feedback.

· Monitor and review participants' budgets and management of their budget in accordance with Support at Home guidelines.

· Ensuring that all documentation is completed in a timely and accurate manner.

· Represent Just Better Care external forums and meetings.

· Identify and participate in personal professional development.

· Ensure compliance statutory and contractual requirements in relation to management of service delivery.

· Manage communication with key stakeholders, internally and externally.

· Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.

· Ensuring that all responsible steps are taken to protect personal safety at work and avoid adversely affecting the health or safety of any other person at the workplace in accordance with requirements under the Work Health and Safety legislation.

· Participating in activities aimed at the continuous improvement of services.

· Complying with Just Better Care's Vision, Values, and Purpose.

· Complying with Just Better Care's Staff Handbook, including relevant policies and procedures.

· Completion of rotation after hours on call service to ensure continuity of support and services.
