Supported Independent Living Team Leader

Position Purpose
The Team Leader is responsible for the day-to-day operational oversight, quality, and safety of service delivery within their portfolio. This role ensures supports are delivered in line with NDIS Practice Standards, individual care plans, and organisational expectations, while actively contributing to risk management, staff leadership, and funding advocacy outcomes within the Support independent living environment.
The Team Leader plays a critical role in ensuring consistent documentation, incident reporting, and proactive risk identification, enabling the organisation to advocate effectively for appropriate client supports, funding, and housing outcomes whilst ensuring appropriate support is provider to staff.
Key Responsibilities
Oversee delivery of supports to ensure alignment with care plans, BSPs, and service agreements
Monitor client wellbeing, changes in presentation, and emerging risks
Support safe implementation of manual handling, medication, and high intensity supports
Ensure clients are supported to achieve their NDIS goals and outcomes
Conduct regular client check-ins and site visits
Ensure all incidents are reported, reviewed, and actioned promptly
Review incident reports for quality, accuracy, and completeness
Identify patterns and trends and elevate where required
Ensure risk assessments are completed, updated, and implemented
Drive proactive risk mitigation strategies across all clients
Maintain oversight of reportable incidents and compliance obligations
Provide day-to-day leadership, guidance, and support to Key Workers and Support Workers
Ensure staff understand and follow policies, procedures, and client-specific supports
Monitor staff performance, provide feedback, and address concerns early including actioning supervision reviews
Support onboarding and training of new staff
Promote a positive, accountable, and safety-focused team culture
Work closely with rostering to ensure supports align with funding, ratios, and client needs
Monitor attendance, shift coverage, and staff suitability
Ensure client notes, charts, and documentation are completed to required standards
Drive compliance with:
Behaviour support requirements
Maintain audit-ready documentation aligned with NDIS Practice Standards
Funding Advocacy & Stakeholder Engagement
Use incident data, client notes, and risk trends to support funding advocacy
Contribute to evidence gathering for:
Increased support ratios
Active nights
SDA/MTA applications
Liaise with Support Coordinators, families, and allied health professionals
Attend and contribute to care team meetings
Selection Criteria
Minimum 2–3 years experience in disability support
Demonstrated experience in a leadership or senior support role
Strong understanding of:
NDIS Practice Standards
Incident management and risk frameworks
Excellent communication, documentation, and problem-solving skills
Qualification in Disability, Community Services, or similar
Additional Requirements
Current NDIS Worker Screening Check
Valid Driver's Licence
Completion of required organisational training
