ACAS positions offer flexibility and autonomy and are based across metropolitan and regional locations, with most roles involving local travel within each region.ResponsibilitiesCollaborating with clients, families and multidisciplinary teams as requiredWorking with colleagues to promote a supportive and positive team cultureConducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support servicesReviewing support plans to meet client's changing support needsWorking autonomously and as part of a broader team to deliver against key performance indicators within identified timeframesMaintaining currency of knowledge relating to funded and non-funded aged care servicesContributing to continuous improvement frameworks to regularly evaluate and enhance assessment processes and outcomesWhat you'll needTertiary Qualification in a health-related discipline related to Health, Aged Care or a related specialist area (e.g. Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)Current unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA)Australian driver's license and access to a registered, insured vehicleMinimum of one year experience delivering services in Aged Care settings and/or to older persons, including people living with dementiaWillingness to apply for a Police CheckWhat we can offerLocation: Mid North Coast with travel within areas such as Taree and ForsterFlexible hours and days – Monday to Friday, part time 3 days per week; no shift workPay rate – $54.47 per hour + superannuation7 additional leave days per yearNot For Profit Salary Packaging to reduce taxTravel reimbursement at 0.88 cents per km in line with travel policyFitness Passport discounted gym membershipsPaid Parental Leave and Long Service Leave after 5 yearsAged Care Assessment Services (ACAS) operate independently of Catholic Healthcare and play a pivotal role in ensuring that Older Australians are assessed for the care and services they truly need.If you're passionate about making a difference and have the experience or skills for this role, we want to hear from you!
