**About the Role**
The AARCS position is aimed at the early identification of older people in inpatient settings who require a coordinated and managed approach to identifying, planning and accessing community or residential care based services on discharge from hospital. The primary role of AARCS Registered Nurse is to assist older people and their carer / families to access appropriate services in order to facilitate a timely discharge from hospital. The AARCS service is aimed at improving the assessment, care planning and discharge planning of older people with complex care needs in the inpatient setting. This position reports directly to the Health Service Manager of Scone Hospital and is fully supported by the team. Ongoing education will be provided by the Clinical Nurse Educator.

The equine focus of the town is reflected in numerous ways: it is the home to the Australian Stockhorse Association; the Hunter thoroughbred breeding industry which is reputedly the second largest breeding area in the world; the district has some of the country's longest established pony clubs; there is a thriving polo and polocrosse community. You will be working with a dedicated team of Allied Health, Medical staff, Registered Nurses, Endorsed Enrolled Nurses and Assistants in Nursing in supporting excellent care to our patients in collaboration with patients, families and carers. You will be supported with a Clinical Nurse educator and a Quality Coordinator on site Monday to Friday.

The successful applicant will be a motivated and experienced individual who possesses effective communication through the use of information technology, written and verbal means with the ability to utilise a range of communication strategies to negotiate resolutions and manage conflict. Your excellent time management and organisation skills will be utilised in both your administration and clinical duties. You will be an adaptable personable individual who has great leadership qualities and skills for assessing and providing support to clients, families and staff.
