If so, we\'re looking for Carer Hub Navigator - who will be the welcoming face and guiding hand for carers in our new Carer Hubs.This is an exciting new role designed to integrate community development and community engagement approaches to raise awareness and enhance accessibility to Carer Gateway services through our Carer Gateway Hub.The Carer Services Navigator role is based in the Townsville region, providing a consistent presence and reliable support to carers, adopting a "no wrong door" approach.
Working in collaboration with a team, you will actively build strong, sustainable relationships with community organisations, health services, schools, aged care facilities and local government agencies to further strengthen and embed carer support locally.The role has four core functions:To create seamless connections between Carers and vital community resources, cultivating meaningful relationships that support wellbeing and independence.Supporting Carers to thrive, focusing on holistic well-being, going beyond basic needs to help carers achieve their full potential.The service is Carer focused, 'how can we help' and a 'no wrong door approach'.
