This role is open to experienced and qualified candidates who are currently residing in Australia with valid work rights.The heart of the roleAs a Care Manager, you’ll lead with purpose - supporting your team and making a meaningful difference to the lives of our residents every day. You will support the General Manager in managing and leading the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance.Duties includeOversee the daily clinical operations of the care home ensuring our residents receive the best care available.Support and lead the care home clinical team including coaching, mentoring, staff allocations and delegation, performance management and education.Review clinical incidents, complaints and changes, providing solutions to reduce any risks.Lead compliance with AN-ACC assessments.Manage person-centred care planning and evaluation for residents - including case conferencing with families and carers.Fostering a strong team environment.Prepare and conduct clinical governance audits.Provide insights and innovative solutions for continuous clinical improvement.Oversee medication management in accordance with resident needs and legislative requirements.Liaise with allied health, medical practitioners and other services to provide optimal resident care.What will you bring?Bachelor of Nursing with current APHRA is essential.Post grad qualification in aged care highly regarded.Strong and effective leadership, communication and interpersonal skills.A person-centred care mindset.A passion for nurturing a strong team environment.Ingrained passion and empathy for our residents.Proven track record in delivering quality clinical care within an aged care or acute setting.Excellent clinical assessment and triaging skills as well as strong care planning capabilities.Proven and demonstrated experience in fostering a reporting culture balanced with your effective investigative skills.
