Kanda is seeking an enthusiastic Home Care Packages (HCP) Coordinator to join our Veterans', Aged Care, and Community Nursing Services team. This full‐time role supports the coordination and delivery of person‐centred care, ensuring clients receive high‐quality services while maintaining compliance with Aged Care Quality Standards.

Your role
* Coordinate and manage Home Care Package services for clients
* Support clients to understand and implement their care plans
* Build strong relationships with clients, families, and stakeholders
* Manage client onboarding, reviews, and service adjustments
* Ensure compliance with Aged Care Quality Standards and documentation requirements
* Collaborate with the Senior Clinical Nurse on service delivery and workforce support
* Respond to client feedback, incidents, and service improvements
* Support growth of the Home Care Package client base
What we are looking for
* Relevant qualification in Health, Community Services, or equivalent experience
* Experience in aged care, community care, or disability services
* Strong communication and relationship‐building skills
* Ability to manage competing priorities and respond with urgency
* Proactive and client‐focused approach
* NDIS Worker Screening Clearance or willingness to obtain
