Competitive Salary & Benefits Full Time Role Based in Goulburn Therapeutic Crisis Intervention and CARE Training Provided The Role As a Permanency Case Manager, you will collaborate closely with team leaders to support children, young people, their siblings, foster carers, birth families, and other key individuals.
Your focus will be on achieving stable, positive, and long-lasting placement outcomes.
You will provide therapeutic case management guided by trauma-informed and evidence-based practices.
Your work will ensure that every aspect of a child or young person's life is supported, and that birth families are empowered to improve their overall wellbeing and functioning.
Where possible, the goal is to preserve or restore family relationships.
When this is not feasible, you will support alternative permanency pathways such as adoption, guardianship, or long-term foster care.
This is a hands-on role involving direct client contact and teamwork.
It includes managing complex cases, offering practical support to children, young people, carers, and families, and fulfilling detailed reporting requirements.
Service delivery occurs in homes, offices, and community settings, and may occasionally require work outside of regular business hours to respond to urgent needs.
