Role Overview
* Safety and Wellbeing: Maintain confidentiality and adhere to organisation policies to ensure participant and colleague safety and wellbeing.
* Partnerships and Collaborations: Develop strong relationships with participants, families, carers, and community agencies to support inclusive and effective service delivery.
* Personal Growth and Development: Enhance skills and knowledge through training, mentoring, and professional development opportunities.
Key Responsibilities:
* Support participants to access and navigate mainstream services and resources, including government agencies and community organisations.
* Assist participants to develop and implement individualised recovery plans, incorporating NDIS funding packages.
* Collaborate with broader systems to ensure seamless transitions and ongoing support.
Requirements:
* Minimum Cert IV in a Community Services related discipline or equivalent qualification.
* Experience in human services, particularly in understanding service delivery and its impact on clients with persistent and complex mental health issues.
* Strong communication, interpersonal, and negotiation skills, with the ability to work effectively with diverse stakeholders.
* Ability to meet deadlines in high-pressure environments and exercise judgment and initiative.
* Excellent organisational and time management skills, with a focus on delivering quality outcomes.
