About the Role
We are seeking a dedicated and passionate NDIS Support Coordinator to join our growing team. In this role, you’ll be at the forefront of helping participants make the most of their NDIS plans—connecting them with the right supports, services, and opportunities to achieve their goals.
Key Responsibilities
•Coordinate and manage services in alignment with participants’ goals
•Strengthen participants’ capacity to navigate and understand the NDIS
•Build strong, trusting relationships with participants, families, and providers
•Maintain accurate and timely progress reports and case notes
•Promote participant choice and ensure service delivery aligns with person-centred best practice
What You’ll Bring
•A genuine passion for making a difference
•Proven experience in NDIS Support Coordination or a similar role
•Exceptional communication and interpersonal skills
•Establish, support, and develop plans to address the identified needs of the participant in line with NDIS plans, individual requirements, and NDIS guidelines.
•Develop and maintain formal and informal networks for participants to maximize engagement and inclusion
•Ensure all supports and interactions are guided by person-centred principles, respecting the individual’s goals, preferences, and autonomy
•Organisational skills with the ability to think critically and solve problems to meet deadlines
•A calm, professional approach, with clear boundaries and empathy
•Background in disability, mental health, social work, or community services
•A current Driver’s Licence and access to a reliable vehicle
•Current NDIS Worker Screening Check and Working with Children Check
•Relevant qualifications in social work, psychology, community or disability services (preferred but not essential)
