About the role
“Community Mental Health Team” (Core Team) is the mainstay of the community mental health system. Core Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.
Consumers under the care of Core Teams will either have:
1. Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
2. A period of treatment as recommended by evidence based practices.
3. Ongoing care and monitoring for prolonged periods.
The clinician will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.

4. Recognised tertiary qualifications appropriate to profession. Full registration and eligibility for membership to professional association, as appropriate.
5. Demonstrated competencies to provide contemporary mental health practice.
6. An unrestricted NSW Driver’s Licence (P2 licence acceptable).

About working for SLHD All NSW Health workers are required to have completed a primary course (2 doses) of a Therapeutic Goods Administration (TGA) approved or recognised COVID-19 vaccine (except for the Janssen COVID-19 vaccine which is approved by the TGA as a single dose primary course). New applicants must have completed the vaccination course prior to commencement with NSW Health, or provide an Australian Immunisation Register (AIR) Immunisation History Statement certifying the worker cannot have any approved COVID-19 vaccines available in NSW. A NSW Health agency may require further information about the medical contraindication (including but not limited to an Immunisation Medical Exemption form – IM011 form). Acceptable proof of vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement or AIR COVID-19 Digital Certificate. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
