Grade 2 Social Worker Acute & Subacute | Regional Public Health Service | Full-Time A large regional public health service in North West Victoria is seeking a Grade 2 Social Worker to join its collaborative Allied Health team delivering services across acute and sub-acute settings. Supporting a large and diverse regional community, this organisation employs 900 staff across a 170 bed health service, delivering a wide range of services including emergency, maternity, intensive care, rehabilitation, community health and mental health programs. This is an excellent opportunity for a Social Worker ready to step into a Grade 2 role, working in a multidisciplinary hospital environment while supporting patient recovery, discharge planning and continuity of care. The Role As a Grade 2 Social Worker, you will provide high-quality, patient-centred social work services across inpatient programs, supporting individuals and families during complex health journeys. Working within a multidisciplinary team, you will provide assessment, intervention and discharge planning support while contributing to the development of the social work service and broader allied health programs. The position works across acute and sub-acute services, providing a varied and rewarding clinical caseload. Key Responsibilities Deliver comprehensive psychosocial assessments and interventions for patients and families Work closely with the multidisciplinary team to support patient care and discharge planning Facilitate continuity of care by liaising with community agencies and external services Participate in ward rounds, case conferences and family meetings Implement evidence-based interventions and contribute to complex care planning Maintain high-quality clinical documentation and data reporting Contribute to quality improvement initiatives and service development About You To be successful in this role, you will bring: A recognised degree in Social Work Eligibility for AASW accreditation Demonstrated clinical skills consistent with Grade 2 practice Strong communication, collaboration and problem-solving abilities Experience working within multidisciplinary healthcare environments Experience supervising students or junior staff will be highly regarded.
